The NHS Health Check is a FREE check up that can help spot increased risk of heart disease, stroke, kidney disease or diabetes early, before the conditions develop. It's an opportunity to find out if you are at risk and to discuss how you can stay healthy.
Whether you have already had a check or not, if you are between the ages 35-75, we would like to know what you think of the NHS Health Check service in Calderdale and how it can be improved.
